Survey for individuals with familial history of genetic diseases

Ms. Nahya Awada, student at University of Liverpool undergoing her Master’s Degree in Clinical Research, invites you to participate in a survey “Knowledge, attitudes, and perceptions of individuals with familial history of genetic diseases regarding implications of personalized medicine to personal health in Canada”.

This is a voluntary and anonymous survey which will take approximately 7 to 12 minutes to complete.

The survey is designed to gauge knowledge, attitudes, and perceptions of families with history of genetic diseases about implications of personalized medicine to personal health and to identify barriers impeding the use of personalized medicine in routine clinical practice in Canada.

This study has been approved by the International Online Research Ethics Sub-Committee at the University of Liverpool.
